The belt did get some extremely minor damage from when the pulley failed. I've kept an eye on it and it doesn't seem to be progressing at all.
I have never changed the oil on the blower, but the blower hasn't been on the car for all that long. Definitely not enough miles to warrant changing it.
The reason I asked about the oil was because if the blower was locking up or having issues, you might find contaminates in the oil

How much tension do you have on that pulley? You may wanna try backing that down a bit. I can't believe you've had this happen three times and there's not something inherently wrong with your set-up. I doubt the shaft is locking up. Catastrophic things worse than the pulley breaking would happen if the shaft were locking up, in my opinion.
The blower came with a larger pulley, so I don't believe there would be any need to adjust the tension on the smaller pulley.

That is a good point. Probably a good spot to start from. I've never changed the oil on a blower, but I can't imagine it would be all that difficult.

the reason the pulley failed was due to over tightening the belt. to much pressure pulling the pulley over just a little like .002. then you drive it around and it flexes the the metal back and forth thousands of times per minute, eventually the metal fatigues and that’s why you get a crack all the way around

The belt was never tightened, and it's a smaller pulley than what was originally on the vehicle.

Belt tension doesn’t really change. The tensioner applies about 90% of the same pressure in the distance of stroke they pivot. They only have about a 70-90° stroke and static clock spring.
