Do you have to hold the start button or just press it once before push start?

If you are push starting you only need to turn on the ignition(dash lights come on). You don't need to try and crank it while the car is being pushed.

Just press the button till the car is in the ON position, then push start as usual. I have done it once on my G37 when the clutch pedal sensor was acting up and the car wouldn't start. Luckily I was in my driveway and I had backed into my driveway. I put the car int he on position via the push start button, took off the ebrake, put it in 1st, and after catching enough speed rolling down the driveway incline i popped the clutch and on she went ;)
