My bet is injector(s) are somewhat gummed up and the spray pattern might not be 100% and causing a slight miss fire or not complete burn in a cyl. causing it to run ruff.
The exhaust sound deeper, After Covid I can hear low tones a lot better than I used to.
Good luck with it.
It most likely just needs an Italian tune up.
you beat me to the Italian tune up suggestion.

Could any of the weights on the exhaust or elsewhere be broken off? The ones that
cancel out vibration?

Well, I got the car in late 2019. Then in 2020 I pretty much promptly caught Covid and was one of those lucky people where it chronically affects you long term. So, that pretty much translated to being stuck in a bed for 2 years. Some days I feel still rather poorly and it really hinders one's ability to go joy riding. Also I do have 5 cars and truth be told I am not a fan of the 350R's hard seats. I love the car, but no, I do not drive it much. I'm not really sure what that has to do with my inquiry, but since you asked.
Wow, and here I was thinking this was yet another GT350R purchased as an investment to resell later.

Hope you get it fixed, and start enjoying it with better health.
