I installed my Pypes long tubes just fine, but the 2 bolts (per side) that attach the cats to my headers keep coming loose. Every time I get under the car they are loose. I think the simple answer is go get some lock washers and re-tighten. I need to do that at some point.

But did I do something wrong at that point? I just took two bolts per side, slid the bolt through the metal plate that pulls the cats into the header, put the nut on and tightened.

Did I do something wrong?

While I have not installed that brand, it looks like the flange is fairly robust, when you have such a robust setup it's hard to get into tension with the bolts. So think of it like this, once you've tightened them good & tight, but cold....once the heat hits them everything expands, then when it cools it contracts, leaving the fastener loose.
A lock washer will be a step in the right direction, you may end up getting a bolt that can take a little more torque. And often I use Anti-Seize, this lets you get more pre-load with less resistance, but watch out it will ruin clothes and get all in your laundry equipment, if you try to wash it out.
Now by the same token you can have TOO rigid of a bolt, you need the bolt to act like a spring, you can see some cars actually have springs on their exhaust bolting for this reason.
I know this was a lot of crap, but I hope this helps give you direction and I do not think that you did anything wrong, but having an open mind to error is a good thing to have.:)
