ATI balancer, how does it go together?

I purchased an ATI balancer from Lethal. It came in 3 pieces and no matter how I put the pieces together it doesn't seem like it is right. Can someone help me with the pieces and how they go together? Here is a picture of the pieces.

Take the bottom right, put over the middle small piece, then put those over the top piece.

I think I may be missing a bolt. I have one bolt hole that does not have a bolt for it. It is the flat head torx bolts with the bevel. There are 6 beveled holes but only 5 bolts that came with it.

Does anybody know if I can run it with a missing bolt or do I need all bolt holes filled?

I would not run it with a bolt missing, if you gave a local fastenal take one of the bolts down and they will match it up for you.
