The classic would be actual heel and toe twisting the foot at about 30-45 degree using the heel of the foot to blip the gas pedal.
I guess if your feet are wide enough you can keep the foot straight up and use the right side of your foot to blip.
I just googled "heel & toe" shifting/braking, and there appears to be many definitions of it. Lots of different ways of doing it.

Well, by definition, it's HEEL and TOE. Otherwise it would be called Left and Right or Side to Side.

 I wear size 10 and no problem doing the "heel&toe"... It took me a bit of practice with touchy brake pedal

Do you own investigation, the name is misleading.

Heel & toe is just a moniker. It is usually the big-toe ball-joint, and right side of the foot. Which is why the after-market makes pedal kits that bring the brake and throttle pedals closer together.

It's not actually expected that someone will turn their foot 90 degrees and actually use their heel & toe. 10 minutes of doing that and pain ensues.

It's what it is. You don't need to turn 90 degree to do reach the gas pedal.
For someone with small feet, that's the only way to reach the gas pedal when you brake hard. Maybe for those with big feet or closer pedals they can just do a parallel blip.
