I've noticed that there is a gap between the bushing and the sway bar's lip to keep it center i believe. Any ideas how this could have occurred ?

Those things move left/right. If the collars on your aftermarket bar aren't tight to the bushings (or perhaps designed with some extra clearance for larger bushings), then ya its going to ride a little to the side and may shift around.

Its not a problem. If it moved too much it would clunk into some part of the suspension, but that's what the collar is there to stop.
