P0161 after service

My dealer replaced my transmission and I'm getting a P0161 (02 heater circuit bank 2) consistently and immediately after I clear it. I do have long tubes but checked all the connections and they all seem fine. I used to get the bank two running lean code after cat but put spacers in and that took care of the problem. I guess the only thing logical is that the rear 02 on bank 2 is shot already? Anyone else experience anything similar?

Problem solved - wire was out of pin lock in harness just enough to not be visible without looking close inside with flashlight. Probably from coming on and off 1000 times but either way for the future P0161 searchers on here that was my issue.....
