Wife and I took our first river cruise April, 2022 in Portugal. 95% was included in the price. Flight, transport to hotel, hotel a few nights, motor coach rides with guides talking about history, curater tours of whatever site we visited, etc. Meals on board plus a few on land all included. Only about 90 total folks on the cruise (boat holds 112 passengers) with 40+ crew. Doing another this October thru 5 countries in East Europe. Bigger boat, 175 passengers max. Not cheap but with so much included and not having to plan/guess where best sites are it's worth it to us. Maybe 5% of time is "Free" time to explore on your own. Of course we can stay on the boat or hit the area docked in vs doing the included tours. There's usually a few other activities offered for a small extra fee if you are inclined. I'd rather be on a SMALL ship vs THOUSANDS on those HUGH ships. But that's me!!'m curious - for trips like this (open to anyone to respond), does one set up an all inclusive tour guide that is there for you the entire time you're visiting?
