When streaming music via Bluetooth from my iPhone 4S, or wife's iPhone 5, the sound system volume must be put to maximum to get a level that is barely audible.

On our Ford Mondeo/Fusion, the Bluetooth volume is the same as the radio (ie doesn't have to be adjusted when switching from one to the other).

Has anyone else had this problem ? Anyone solved it ? (I can't see a setting to change the Bluetooth volume level in either the car nor an iPhone.)

Maybe your phone volume automatically lowers the first time you pair with a new device.

^^^ This...Assuming you have your device volume turned up..

Some media is cleaner/louder than others too. I am not overly impressed with the Shaker systems though. Oddly enough, the Sync II in my Wife's 2013 Taurus will blow my Sync II/Shaker nine speaker out of the water as far as volume for some reason.

For reference, I am usually using a Apple 5s.

I would bet its what Niz and Lethal said. Most phones have a volume setting for ring tone, notifications, media, system, and Bluetooth. Once connected and playing musice turn the volume up on side of phone like you would do for the ringer volume. This should turn up your Bluetooth volume as long as you are connected and music is playing.

I have a Samsung 6, when my radio does this the phone volume is down low, just turn the volume up on the phone.

I purchased my 2016 recently and have no issues with Bluetooth volume with an iPhone 6. I mostly stream Pandora.

the suggestion to simply turn-up the volume on my iPhone was (ie the bleeding obvious:)) was the (simple) solution! (I was expecting it to come out of the phone at a fixed level - like the Line Out on the old analogue stereos)

Don't over-think things :)

I have the same issue streaming music via bluetooth with my android galaxy S6 however, if I connect my iPod hard wire and play the same sound, it sounds way better.
