Grinding from 4th to 5th gear.

About 2 months ago I noticed a terrible grinding noise from 4th-5th so I took it to Ford under warranty. They told me synchros had gone bad so they told me they had to basically rebuild 5th gear and fix the synchros. Got the car back and everything was working fine and then about 200 miles later it started to grind again from 4-5. What could be the issue? How can the synchros already go bad after 200 miles? Could it be something else like clutch drag?

The plastic riders on the shift fork maybe. Bring it back, tell em to fix the issue PROPERLY or replace the trans. Settle for nothing less.
