I have a 2016 Mustang GT, and I’ve recently been having some AC issues. It first started months ago when I discovered there was a hole in my condenser (which I did replace), and that fixed the problem for now, until about 3 weeks ago, where my AC wouldn’t work and was blowing out warm air. I tried to recharge the system, and it worked for a couple days, then started blowing warm air again. I then recharged it again a couple days ago, and it only lasted a 15 minute drive home before the rest leaked out while it was sitting, and now when I turn the AC on, I hear a hissing noise coming from behind the dash like it’s out of refrigerant, what could the issue possibly be?

It could be multiple things. It seems clear you have a leak in the system, and the location(s) have to be found first. It could also be the evap core behind the dash, based upon your post information.

When mine failed, it was the compressor. Once that was replaced, I was back in business again.

Refridgerant leak detectors are available for purchase. If you've been replacing components yourself and want to continue, might be worthwhile to add one to your toolbox.

Google, "R134A Leak Detector".

Another tool which might help are tracing dyes. Others with experience could provide insight on these.

Edit: the condenser (or any one part failing) I can understand. Fixed. Check. But now with yet another leak it a suspected second area...any chance something was left loose or came loose? Were O-rings inspected or replaced when the condenser was?

As far as I know, nothing was left loose, I did inspect the o rings at the condenser and they were pretty good

Your O rings are 7-8 years old. They should be replaced.

Based on your description my armchair guess is a leaking evaporator. It's a known, common problem on 15-17 mustangs.
Anytime you replace an ac component you should replace any o-rings that were distributed. If the system is discharged you should also replace the Schrader valves.
When you replaced the condenser did you place the system in a vacuum before you recharged it? If not you left a crap load of moisture in the system that could have exacerbated a failing evaporator.
If it is a leaking evaporator the dash has to be removed to replace it. A $1600 to $2500 job at a shop or a multi day diy job.
If you decide to diy then spend some time learning how the ac system works and buy yourself a vacuum pump and a set of manifold guages and familiarize yourself with how they work and what they mean.

Meh, I usually recover the system then pressurize it with nitrogen to locate leaks.
