I've had one in both my EB and my GT and they're not bad at all. I'm also not sure what "sluggish, heavy and unpleasant" mean in relation to the MT-82. They seem to be none of things to me. I've driven my fait share of manuals and I do not mind the the MT-82 at all.
No problem at all; They probably presented it as a package deal like most dealers do, but they'd be two separate purchases on the bill of sale.
As a note, I had bought both on my EB, but when I purchased my GT last week they gave me a prorated refund which I applied to the cost of the car. So...

PremiumCare ESP and the Premium Maintenance Plan through Ford are two different things. One is an extended warranty and the other offers Oil Changes et. al. They are sold separately. This thread specifically is about the ESP not the Maintenance Plan.

I kept my 15 Ecoboost PP for almost exactly 3 years. I purchased 11/2014 and traded it yesterday for a GT. The Ecoboost was fine, and with PP it handled really well. It just wasn't fast enough for me.

Correct. Couldn't be easier. Unplug the light from the car harness, plus the new resistor into the harness, plug the light into the other end, and screw back in as normal.
The longest part of this install will be popping the fasteners to get to the lights.
