I got mine from tasca on ebay - the only thing extra I had to buy when bought it from them was the pins and bracket but depending on what pads you buy it should come with it.
There's some take offs or used ones you can find that would probably be cheaper -

I know what you're talking about. Have you tried maybe unscrewing the bolt and aligning the nut right behind the adapter and re screwing it back in holding the nut down?
Forgot to update my final result. I ended up returning the 15 TB and getting a 13 TB and it worked with no issues. She pulls! Still debating if I'm going to port the manifolds....hmmm

I just reminded myself right now that the first video was with the previous tune loaded on, the 2nd video is with the tune revision which when you put the car in gear and drive, then the wrench comes on and car runs like sh*t.
