Voltage readings on O2 doesn’t look right

This is taken from livelink SCT
why is "bank 2 rear" at 0 Volts?
why is there a bank 3?
Shouldn't it just be bank 1 front, bank1 rear, bank2 front and bank 2 rear?
Are the volts moving like they should?
