That is a lift, not floor jacks. He is not the first to do that and wont be the last. Hopefully no one was killed.

Everytime i go under a car on a lift i cant believe i havent seen that happen ever in person and ive been working in car dealerships for over 20 years

Was not a mustang but a good friend of mine had that happen to his work F-150. Was in for a routine oil change and they dropped it. Not a pretty sight at all....

How does that even happen? Is it a hydraulic failure? I thought there were mechanical backup fail-safes if it was a hydraulic issue.

It can happen if you lower the car but there is something under it, like a transmission jack, and that causes the car to not lower evenly and slide off. Other times the contact points for the jack were not secure or oily and your wrenching caused it to slip. List goes on. Technically, you want to lock the arms in place such that they cannot move even without weight on the car, but many jacks only have a pin system that restricts where you can put the arm to make contact with a jack point.

Curious - was that S550 already previously wrecked - there's no front bumper cover and I'm not seeing the rocker panel cover either under driver door?

Mmmnnn... rather than Workshop ..... I suspect Photoshop (by the odd way it seems suspended) - although the door panel has an upward impact.
